After nearly 27 hours in a marathon operation, doctors at Melbourne's Royal Childrens Hospital have successfully separated conjoined twins Trishna and Krishna.

Surgeons at Melbourne's Royal Childrens Hospital began the surgery to separate the two-year-old twins, who are joined at the head, about 8.30am (AEDT) on Monday.

The Bangladeshi orphans arrived in Australia just over two years ago through Moira Kelly's Children First Foundation.

Foundation Chief Executive Margaret Smith said she tried to remain calm as surgeons worked to separate the twins' brains millimetre by millimetre.

"The wait has been very drawn out and it has been very stressful," she told AAP.

The hospital's head of surgery Leo Donnan praised his doctors, who he said had worked seamlessly to separate the girls.

"This is a once-in-a-lifetime operation that teams would do," Mr Donnan told reporters outside the hospital.

"There really still is a long way to go," Mr Donnan said.

"The girls have a very difficult time ahead of them."

"For the hospital, it is an historic moment and for the girls, an even more historic moment."

The operation went for longer than would have been expected, Mr Donnan said.

He said the staff had taken planned breaks and the twins were well nourished before the operation.

The mood inside the operating theatre had changed following the separation, Mr Donnan said.

"It's been a very nice stage to move into," he said with a smile.

CONGRATULATION, Medical team are HEROS! Our hearts and prayers are with the girls and those who care for them, especially in the next 24-48 hours. Their girls' operation brought back lots of memories when the Bosin twins, Eustina and Eusthosia were separated in the same hospital 12-13years ago. My husband and I were privileged to play a major part in the Bosin twins, who are now in their early teens, healthy and enjoying school back on Buka Island, Bougainville in PNG. Best Wishes, Sana.
