Technology World - Your mobile phone number could be hijacked

Technology World with Dr Ka Fung: Australian journalist, Tracey Holmes, recently reported her mobile phone number was hijacked. Her phone number was apparently ported without her permission. Could this happen to any of us and what should be done if this happened ?
