Queensland's coroner James McDougall's inquest findings were damning.

In his more than 300-page report he said there was no evidence Dreamworld properly maintained the ride in the three decades it was open to the public.

The coroner has delivered scathing inquest findings over the deaths of four people at the Gold Coast’s Dreamworld amusement park in 2016 and referred them to the regulator for possible prosecutions.

He said a systematic failure in management at Dreamworld led to the operation of the Thunder River Rapids Ride for years with little consideration for the safety of patrons.
