A Melbourne couple, missing for four nights in Victoria's rugged alpine region, is having a hospital check up after emerging from the bush.

Police said Trevor Salvado, 60, and Jacinta Bohan, 58, were "safe and well" when found on Tuesday by a kayaker at Buffalo River, in northeast Victoria.

The pair has since arrived at Wangaratta Hospital, where it is reported they are being treated for exposure and dehydration.

Footage from the hospital showed Mr Salvado walking unaided from an ambulance into the hospital, followed by Ms Bohan on a stretcher.

"We haven't seen them but they're well. They walked out," an emotional friend told the Nine network.

"Their kids are great - they're seeing them now."

The couple endured frosty conditions after disappearing while hiking at Mount Buffalo on Friday.

Their disappearance sparked full-scale search, involving up to 150 personnel covering scores of kilometres of rugged terrain.

The couple had been expected back from a bushwalk at Reservoir Track on Mount Buffalo about 12.30pm on Friday, but did not return.

Their car was found the next day.

Victoria Police thanked search crews and the public for their assistance.

Earlier, Jacinta Bohan's sister, Maria, said the couple had been visiting the area for eight years.

"They're very healthy. They're used to being out for hours and walking, and, you know, they spend a lot of time outside," she told the Nine Network.
