Lindt cafe siege deaths ‘not the fault of police’, NSW Coroner's inquest finds

NSW Coroner Michael Barnes is handing down his long-awaited report into the 2014 Lindt Cafe siege.

Coroner Barnes acknowledge the pain of the Dawson and Johnson families would not be reduced by any changes in the response to future terrorist events.

He also acknowledged those who had contributed to the 23-week long inquest and made his final findings.

"I confirm that the three deaths investigated occurred and I find that the first person who died in the incident was Tori Johnson," he said.

"Mr Johnson died on December 16, 2014. He died in the Lindt cafe, Martin Place, Sydney, New South Wales.

"The cause of his death was a gunshot wound to the head. Mr Johnson died when a person who had held him and others hostage in the cafe intentionally shot him in the back of the head with a shotgun at close range. Tori died immediately.

"The second person who died was Man Haron Monis. Monis died on December 16, 2014.

"He died in Lindt cafe, Martin Place, Sydney, New South Wales. Because of his death multiple gunshot wounds. Monis died when people stormed the cafe where he had been holding hostages and they returned fire after Monis fired at them as they entered.

"The police officers who shot Monis reasonably believed that was necessary to protect themselves and others in the cafe. 

"The third person who died was Katrina Dawson. Ms Dawson died on December 16, 2014. She died in the royal prince Alfred Hospital in Camperdown, New South Wales.

"The cause of death was gunshot wounds. Ms Dawson died when police stormed that Lindt cafe at Martin Place in order to free her and others who had been taken hostage by an armed person.

"A bullet or bullets fired at that person by police officers ricocheted off fragmented and accidentally struck Katrina as she lay on the floor seeking safety, mortally wounding her."
