The report, commissioned last year, says unrealistic targets led police officers to manipulating breath test devices to falsely inflate the number of tests conducted.
There is no suggestion any drivers have been wrongly prosecuted.
The review says many front line officers and senior command members were not aware of the practice.
The review makes 23 recommendations, which Victoria Police has accepted.
