The resurgent Australia forward was crowned player of the year and was voted the players' player of the year. He also earned the club's Golden Boot award for his 13 goals in the A-League regular season.
Midfielder Roly Bonevacia was named under-23 player of the year.
Burns's awards acknowledge the attacking contribution of the 26-year-old, whose career appeared to have hit a speed bump in recent seasons in Greece and South Korea.
He returned to the Socceroos courtesy of a goal haul which is second only to the 16 of Sydney FC's Marc Janko in the A-League.
Burns became the first Phoenix player to score a hat-trick in seven seasons during its 5-1 thrashing of Melbourne City in Wellington on 1 November.
It plays the same opponent at the same venue in an elimination final on Sunday.
