The results of a recent research show that a few months after discharge, if there is constant communication between the patient and the nurse, then the possibility of the patient being hospitalized again is reduced. Health experts share how current limited workforce in the healthcare industry makes this 'targeted care navigation' inaccessible for most of the patients.
