Report finds police took too long to storm Lindt cafe after gunman Man Haron Monis fired first shot

It says police commanders had underestimated the threat Monis posed, in part because of flawed advice. But the report says, while police made mistakes, they should not share the responsibility for the deaths of two hostages.
