A man accused of stabbing a fellow worker to death at a western Sydney building site has not slept or eaten for two days, a court has heard.
The 54-year-old victim, reported to be a bricklayer, was found by police with multiple stab wounds after they were called to the site at St Clair on Monday morning.
Paramedics tried to revive the man but he died at the scene.
His alleged attacker, 41-year-old Chun Huang, turned himself into Campsie police a few hours later.
His lawyer, Anthony Goodridge, told Burwood Local Court on Tuesday that his client needed an urgent medical assessment because he hadn't eaten or slept for two days.

Huang, who was wearing a white and blue tracksuit and needed a Mandarin interpreter, appeared worn out in court.
Magistrate Christopher Longley accepted the request and adjourned the matter until Wednesday at the same court.
