The 116 Hot Issues Briefs released in Perth on Friday include 58 related to personnel behaviour such as inappropriate sexual conduct, 16 related to capabilities and 42 covering other matters such as equipment loss and reports.
Mr Smith said 67 of the incidents were reported to civil police or the Australian Defence Force Investigative Service, while three were reported to the Defence Security Authority and relate to the potential theft of documents.

Mr Smith told reporters 245 of the 248 Hot Issues Briefs submitted to him had now been released to the public.
"Three are being withheld at this stage because they may prejudice either ongoing operations, prejudice investigations or lead to invasion of privacy, so they will be released in due course," he said.
The Hot Issues Briefs provide early advice to the minister's senior leaders on matters and incidents that may require immediate attention.
Defence has progressively made public the Hot Issue Briefs at the request of media and Freedom of Information applications.
Mr Smith said that to enhance transparancy he had instigated a reform for briefs to be released publicly within a week of them being submitted to him.
He said briefs would be excepted if they endangered operations, prejudiced policing investigations or prejudiced legal proceedings.
