An audit of a NSW courier haulage company involved in two fatal crashes in the past four years has found "significant breaches".
Barnetts Couriers in Wollongong was raided by police and Roads and Maritime Services inspectors on Friday to audit 16 vehicles.
Two major defect notices were issued for defective lights and seatbelt offences, while the joint traffic task force reported "significant breaches" relating to the company's information systems.
There were no medical records kept for heavy vehicle drivers, limited other driver records and no fatigue management procedures or records.
Vehicles were also found to not have operational brake lights.
The raids come after a woman in her 60s was killed when a truck ploughed into the back of a car stopped at a red light in Barden Ridge on Monday.
"In the event of a serious injury or fatal crash involving a truck the task force will look closely at all aspects of those involved in driving, loading or operating vehicles involved in these offences," Assistant Commissioner John Hartley said on Friday.
He said heavy vehicle safety should be a high priority for the transport industry.
