Probes into how system failed toddler

The violent death of toddler Mason Jet Lee has sparked health and child safety reviews as well as a homicide investigation.

The actions of doctors and child services officers are being investigated following last month's violent death of Caboolture toddler Mason Jet Lee.

The 21-month-old died on June 11 - three months after he was reportedly hospitalised with a range of injuries including a broken leg, severe nappy rash, a ruptured bowel and an anal tear.

Doctors suspected long term abuse and alerted child protection officers but he was fully discharged and sent home.

Health Minister Cameron dick has called for a director-general's report into the actions of clinicians and how the system handled Mason's case.

Child Safety Minister Shannon Fentiman said an independent expert panel was also reviewing the tragic death and promised all their recommendations would be implemented.

Detectives investigating the toddler's death last week said Mason had suffered "horrific" injuries from head to toe.

Acting Inspector Ben Fadian said Mason was sick or injured for up to a week before he died and injuries were inflicted over a sustained period of time.

Opposition child safety spokeswoman Ros Bates claimed the number of Queensland cases of child abuse were blowing out and the state was on the verge of a child abuse crisis.

"The fact that Mason was returned back to his family is horrendous," she said.
