The gunman and two hostages have been confirmed dead following a 16-hour siege at a Sydney cafe.

The Lindt cafe's 34-year-old manager Tori Johnson was shot dead in the final minutes of the 16-and-a-half hour standoff.

Also killed was barrister and mother of three Katrina Dawson, 38, who worked in the city.

Heavily armed police stormed the Martin Place cafe early on Tuesday morning. The 50-year-old gunman, self-described Muslim cleric Man Haron Monis, was also killed.

Police say a "confrontation" occurred about 2.10am between police and the gunman, who'd taken 17 people hostage.

Survivors:

Out of the 15 surviving hostages, five ran free on Monday afternoon, another four ran out prior to gunshots being fired early on Tuesday morning. Police have confirmed that seven hostages were rushed to hospital including two pregnant women. Also among those injured in the siege was a male police officer, who suffered a facial wound from gunshot pellets.

Here's what we can confirm about the survivors:

Distinguishable in his blue blazer, John O'Brien, 83, was the first hostage able to escape from yesterday's Martin Place siege. Dramatic video footage from the event shows Mr O'Brien exiting the Lindt Café site seconds ahead of another hostage.
