The deaths of seven babies at a hospital in Melbourne's west are being investigated.

2006-2013 - Births at Djerriwarrh Health Service almost double

FEBRUARY 2013 - Doctor complains about Surinder Parhar after his patient loses a baby. Investigation starts. The health department is advised

2013 - Seven babies die, five deaths avoidable

2014 - Three babies die, two deaths avoidable

JANUARY 2014 - Australian Nursing and Midwifery Federation take concerns about clinical risk, understaffing and the lack of policies to Djerriwarrh maternity service's director and the Department of Health

2015 - One unavoidable death

MARCH 2015 - The health department learns of the cluster of baby deaths, and orders review

JUNE 2015 - Investigation into Dr Parhar finishes and conditions are put on his licence

JULY 2015 - Dr Parhar retires

AUGUST 2015 - New interim chief executive appointed to Djerriwarrh

OCTOBER 2015 - Djerriwarrh loses confidence of health minister and is to be replaced

OCTOBER 2015 - Family of child born with hypoxic brain injury to launch legal action, others who lost children also considering options
