Culture of carelessness precipitated fatal Optus triple zero outage

Parliament has been told of the chaos at the offshore call centre used by Optus during last year's triple zero outage. Explosive testimony from the author of the review into the deadly failures blasted a culture of 'carelessness'. Representative from the Singapore-based parent company 'Singtel' were also called to account.
