Optus blames human error for fatal Triple-0 outage as Communications Minister warns of consequences

Telecommunications giant, Optus has blamed human error for a Triple-0 outage that resulted in the deaths of three people. Communications Minister Anika Wells has declared there will be consequences after holding talks with Optus executives in Sydney today.
