Optus confirms three deaths after triple-zero calls blocked during network outage

Optus' CEO says a technical failure occurred during a network upgrade that impacted hundreds of customers trying to make 000 calls in South Australia, the Northern Territory and Western Australia. Optus discovered through its process of conducting welfare checks on those impacted that three people at households where calls had been made had died.
