WA Coroner blames Department of Justice failures for Indigenous teenager's death in custody

A WA coroner has ruled the state's first youth death in custody should never have happened, blaming deep failures within the Department of Justice. He urged Unit 18 be shut down as a matter of urgency. but the state government says it won't. WA Correspondent Christopher Tan reports. And a warning this story contains details of self-harm.
