Former State of Origin rugby league player Ben Ross, who snapped his right arm in a live TV charity arm wrestling competition, says it's the most painful injury he's ever had.

And this is coming from a burly bloke who once broke his neck in an NRL game.

Ross and fellow former NRL player Wendell Sailor went face to face in the contest on the Nine Network's The Footy Show on Thursday night, to raise money for the Men of League charity.

Before the live segment, Ross warned Sailor that he had a tight shoulder.

"He said if it feels like it's going to go, I'll just tap out and `let go'," Sailor told the Kyle & Jackie O radio show.

Graphic footage of the incident, which shows Ross screaming in pain before host Paul Vautin cuts to an ad break, has gone viral on social media.

"Go to a break. We need an ambulance," yelled Vautin.

Ross has spoken from his Sydney hospital bed, saying he heard a snap and noticed his arm at a different level.

"When I looked at Wendell's face I knew something was wrong," he told the Nine Network.

"His face, if it was possible, turned white."

The former NRL prop, who will undergo surgery for a broken humerus on Friday, admits the injury hurt.

"This is probably the most painful thing I've ever had," said the 2005 sexiest man in league.

"It's something I suppose I'm accustomed to from playing footy with injuries and that kind of thing."

Sailor has since apologised profusely.

"Even if you didn't like someone you never want to do that to someone.. just watching his face, hearing that noise go snap .. it was just terrible," Sailor said.

But the 35-year-old Ross has been in good spirits, tweeting from his hospital bed.

"Wasn't anyone's fault mate. @NRLFootyShow were amazing with their help and care last night."

He's even already asked for a left-handed rematch.

"It was an awful moment for everyone," said The Footy Show's executive producer Glenn Pallister.

Ross came out of surgery on Friday afternoon.

"Ben is now out of theatre and recovering," said Ron Coote, chairman of the Men of League Foundation, where Ross works as welfare and education officer.

"This has been a very public episode for Ben and now it's time for him to recuperate."
