SBS World News Radio: The leader of a police tactical team positioned outside the Lindt Cafe during the Sydney siege was under the impression his unit was not to storm the building even if a hostage was killed.

Police witness admits confusion over action plan at Lindt cafe

The inquest into the 2014 siege heard that there was confusion among some police about the triggers for storming the cafe and initiating the so-called Emergency Action Plan.

Police stormed the Martin Place building at about 2.13am on December 16, more than 17 hours after the siege began, when gunman Man Haron Monis shot dead cafe manager Tori Johnson.

Monis was killed by police, while Ms Dawson died after being hit by police bullet fragments.
