Primary Health Care chief executive Peter Gregg will be replaced by Malcolm Parmenter in September, with chief financial officer Malcolm Ashcroft filling the role until then.
Mr Gregg, who was also managing director, announced his resignation in January after being charged with falsifying records while an executive at Leighton Holdings but had stayed on until a successor could be found.
"This year, in difficult circumstances, Peter has continued to bring his usual passionate and dedicated approach to his duties and acted in the company's and shareholders' best interests," Primary Health chairman Rob Ferguson said.
