A teenager has died after a car crashed through the front of a chemist shop in Sydney's south.

A four-wheel drive veered off Railway Parade in Kogarah and smashed through the glass shopfront of O'Reilly & Daly Chemists on Monday morning, a NSW Fire & Rescue spokesman said.

At least three other people are injured and on their way to hospital, the F&R spokesman told AAP.

Paramedics are treating a fourth person at the scene.

The family inside the car managed to get out safely but a man in his 60s was pinned underneath a cabinet inside the chemist.

A 10-tonne winch was used to lift the car away from the cabinet and the man was taken to hospital, NSW F&R superintendent Tom Cooper said.

A woman, who was also in the chemist, was injured by glass and had to be pulled clear of debris.

The female driver of the 4WD was taken to hospital suffering shock.

Monday's accident occurred in the same spot where a young woman died and at least 11 people were injured in 2007.

Fashion student Emma Hansen, 20, was killed when a car clipped the kerb, mounted the gutter and hit people who were waiting for a bus next to the chemist.
