Tensions remain high on a remote Northern Territory island after two men were killed in a violent brawl.

The two related men, aged 19 and 29, died at a local clinic after they were hit with spears in Angurugu township on Groote Eylandt on Friday night.

While no charges have been laid, police say CCTV footage appears to show one of the deceased men throwing a spear that fatally wounded a male relative.

That man was then speared by others involved in the brawl, police say.

"Police believe the man who threw the spear is the second male who was killed," he said on Sunday.

"This is an extremely volatile situation which has been compounded by the death of another person on he island unrelated to this incident."

He urged those involved to make contact with police so they can be protected from revenge attacks.

The brawl started as a relatively minor dispute between two related women on Thursday night.
