A Melbourne health service has been charged after the death of an elderly patient.
Melbourne Health has been charged with three breaches of the Occupational Health and Safety Act following the 75-year-old man's death at its Broadmeadows aged persons mental health unit on September 23, 2013.
WorkSafe alleges Melbourne Health contravened the Act on three occasions when it failed to ensure, so far as reasonably practicable, the patient was not exposed to risks to his health or safety.
