The accidental early release of three West Australian inmates has prompted a review of processes between the state's courts and prisons.
In one instance, an inmate on remand at Hakea was sentenced to imprisonment for one of several charges when he appeared in court via video link but he was instead set free and reportedly banged on the door of his ex-partner's house.
Corrective services minister Fran Logan said the errors were detected quickly and all three were promptly returned to prison.
"I believe one or two of the incidents were as a result of problems with paperwork from the courts and one was the responsibility of the Department of Corrective Services," Mr Logan told reporters on Friday.
He said processes between the courts and prisons were being reviewed and improved.
"The commissioner and the director general have just advised me that they are now putting in place both at the courts and at Hakea a far better system to ensure we don't have those issues arise again," the minister said.
A department spokeswoman said training for court and prison staff were also being investigated to ensure the mistakes were not repeated.
