The Telstra CEO has apologised for an outage that prevented 148 triple-zero calls from connecting with emergency services.
One of those calls was for a Melbourne resident who was experiencing cardiac arrest. He died.
Telstra says it’s internal review will be “swift and forensic” in determining the root cause of the outage, while the telecommunications regulator ACMA is assessing whether there have been breaches of legal obligations.

She told SBS's Biwa Kwan while technical failures cannot be completely avoided, it is important that a clear and effective plan is in place to mitigate the impacts.
